When you first arrive and meet our staff, you’ll fill out your new patient paperwork. This helps our team get to know you better. We ask that you arrive 15 minutes early to complete your paperwork. Or, you can get a head start by filling out your patient forms now. Download New Patient Forms.
New patients often ask if they can get a refraction (checked for glasses) during their first visit. Your first visit usually begins with a medical visit. Soon after a refraction would be scheduled as soon as the next day. This would be depending on your insurance coverage and the physician’s prior evaluation. Again we need to see you first for a comprehensive exam and then schedule you for glasses.
We accept all major insurances.*

In order for us to provide you with a copy of your medical records, you must sign a Release of Records form. This can be downloaded from the Forms page on this website or from our office. It is important to note that once you have executed this form it can take 10-14 days for us to copy these records for you. In addition, because of the labor involved, we must charge a fee for this service. Please be sure to determine the specific cost for your record at the time you request this copy.
You may call our office or use the Rx Refill. When calling please make sure you have your pharmacy information available. If you end up using our voicemail please speak clearly and slowly making sure you specify your name, a phone number, and the medication you are requesting to be refilled, pharmacy name, and pharmacy phone number. Please note that prescriptions are not automatically refilled. It is up to the doctor to decide whether or not to refill the prescription. Our doctors are very busy and are not always available to immediately respond to these requests.
We see patients of all ages in our practice. Children should have their first exam prior to their 2nd birthday.
It is important that you notify our office as soon as possible when you know you are going to miss a scheduled appointment so that we can make this slot available to someone else. In the event that you notify us less than 24 hours in advance we charge a fee of $25. Cancellations less than 24 hours on surgery day will be charged $100.
Unfortunately we don’t perform Lasik procedures at our practice.
